INSTRUCTIONS FOR ACCEPTED APPLICANTS


1. Get referral slips from Ms. Myrna T. Palencia starting Friday, May 20. Confirm your intention to enroll in BS Tourism on or before Wednesday, May 25, 2005. Your slot will automatically be given to waitlisted applicants if you fail to confirm before the deadline. You may confirm by email (address: ois.ait@up.edu.ph) or by phone (tel: 9269074/ 9283327) to Ms. Chad Guerrero and/or Ms. Palencia.

2. Present the referral slip to your former college and secure:

2.1 Permit to transfer (2 copies)
2.2 College Clearance (3 copies)

3. Submit to the University Registrar’s Office (Admissions Sections) the original copies of your permit to transfer and college clearance and get University Admission Slip.

4. On Thursday, June 2 , submit to AIT (c/o Ms. Myrna Palencia) the following:

4.1 University Admission Slip
4.2 Permit to Transfer
4.3 College Clearance (2 copies)

Ms. Palencia will issue your Form 5A, the pre-enlistment form . If you have pre-enlisted in your former college, get your CRS result.

5. Proceed to enrollment adviser for advising. For UP students present your CRS result.

6. Enlistment. Enlist in courses advised by the enrollment adviser. After having been enlisted in all the courses you need, get Form 5 and fill up all the necessary data/information.

7. Proceed to designated areas for:

7.1 Post Advising
7.2 Checking
7.3 Assessment
7.4 Payment
