27 May 2003 Memorandum No. ERR-03-068 To: ALL CONCERNED Subject: Check Replacement Fee ------------------------------------------------------ Please be informed that effective May 23, 2003, the University will charge a fee of FIFTY PESOS ONLY (P50.00) for every replacement of any check that is lost, cancelled, destroyed, or has become stale, except when the cause of the replacement is the result of an official decision or action of the University. This is being done to encourage people to secure their checks and to encash or deposit their checks on time. A check is rendered stale six (6) months after its date of issue. We should be aware that the University spends money every time a check is replaced. The expense s involved in preparing and processing a check include not only the cost of checks and supplies but also the cost of man-hours spent by all those involved in the process. Application for replacement of checks will only be processed if accompanied with an Official Receipt showing proof of payment of this fee.
