To: All Deans, Directors, Head of Units, Faculty and Staff

Subject: Application for retirement/resignation/transfer

Attached is the new application form for resignation/
 retirement/transfer, which must be submitted to the 
 Human Resources Development Office at least a month 
 before the effectivity of such request.

Please take note that all applications must include 
 duly approved University clearance. For retiring professors, 
 curriculum vitae must be attached.

For immediate implementation.
