Subject: Request for Information on Faculty Resignations

We are doing a study on the reasons why faculty members resign 
 from the University. To enable us to make an initial assessment 
 of the situation we request your office to provide us information 
 on faculty resignations in your unit/college in the last three 
 (3) years, with details, if available, on the reasons for the 
 resignation,

I would appreciate getting your report not later than October 31, 2004.
