How to secure an Official Transcript of Records (OTR) and other documents, such as...?

HOW TO SECURE AN OFFICIAL TRANSCRIPT OF RECORDS (OTR) AND OTHER DOCUMENTS
  • ~ Secure an application form at the TORS (OUR) counter.
  • ~ Fill out the application form properly and completely .
  • Use the name based on your birth certificate; For married female student, indicate the family name you used during your last enrollment in UP. If shiftee, indicate the college(s) last attended.
  • ~ Submit your accomplished form at the TORS counter for processing and assessment.
  • ~ Pay fee at the Cashier's Office near PNB.
  • ~ Submit at the TOR Counter :
    1. a ) Properly accomplished form
    2. b) Official Receipt (OR) of payment
    3. c) One (1) piece of 2x2 ID picture, black and white or colored (optional for non-board applicants)
    4. d) University Student Clearance, if you have not submitted your University Clearance based on your last enrollment in the University. However, if you are currently enrolled, you need not accomplish a student clearance.

  • How to apply for an English Translation of Diploma (DT)?
  • ~ Present the original Diploma and a photocopy of it .
  • How to apply for CAV ?
  • ~ For CAV (Certification, Authentication and Verification), original or photocopy of documents to be authenticated.

  • How long does it take to process Transcript of Records and Other Documents?

  • How do I claim the documents that I applied for?
  • ~ Present claim stub / official receipt (OR) and identification card. When claiming for a third party, submit a letter of authorization together with an ID card from the owner of the document(s) to be claimed

    1. ~ 20 US$ [max of 3 sets] inclusive of mailing fee via registered mail

    4. ~ A student may opt to send the requested documents through the following modes provided an additional fee should be added:
      1. - EXPRESS MAIL - US$20 [Total of US$40]
      2. - COURIER - US$30 [Total of US$50]

    5. How to file your application via mail or email?
    6. -Mail
      1. ~ Send a written request with the following information :
        1. - complete name used when last enrolled in the University
        2. - year of last enrollment and the college where last enrolled
        3. - course or degree earned from the University
        4. - number of copies requested
        5. - mailing address
      2. ~ Attach an approved student clearance for updating or 1st time application
      3. ~ Include payment in the form of postal money order payable to the Office of the University Registrar, UP Diliman with mailing address:

      3. Points to remember : Students / Alumni of the following Contituent Units (CUs) may apply in UP Diliman from the period stated below :

      6. UP Visayas - up to 1983
      7. UP Clark - up to 1992

      9. TOR's and other documents not claimed within one (1) year are subject for disposal.
