Physical Examination For Freshmen
1. Present your letter of admission at the Record Section.
2. Record Section will give you the necessary forms to be
properly filled. There is a consent form for physical/medical
examinations. This shall be signed by the parent or guardian.
3. Have your Chest X-ray.
4. Go to Dental, Public Health for blood pressure, height and
weight and vision testing.
5. Come back after 2 working days for the X-ray result.
6. Return your green or yellow form to Record Section.
PLEASE DO NOT BRING IT HOME.
7. After 2 days - go back to Record Section - get form with
X-ray result. Present the signed consent.
8. Go to Triage for direction for your physical/medical
examination by a doctor - male student to male doctor and
female student to female doctor.
9. Go back to Record Section for your medical certificate.
Back of X-ray result will be stamped - fit to enroll.
For Medical Check-up
1. Go first to the Triage for instructions
2. Get your medical records from the Record Section.
3. Return to Triage for directions and doctor assignment.
4. Go the cashier to pay necessary fees.
5. Submit to the Record Section the medical findings.
