The Masters Application Form is available at the right column of this page. Applicant information entered in this Form should be sent by e-mail to mba@up.edu.ph. The instructions for submission of applicant information by e-mail are enumerated in Section B below. The submission  of applicant information is just one of the requirements for admission to the UPCBA Masters program. The admission requirements for the academic year 2009-2010 are given in Section C of this page.

 

The application procedure requires the use of the free and downloadable Adobe Reader software to complete the Application Form. Please download and install the latest version of this software first (if it is not yet installed in your computer) before proceeding with the application process. The installation file of the latest version of Adobe Reader can be downloaded here.

 

A. Application Process

 

The three stages of the application process to be accomplished in sequence are:
 

  1. Complete the downloadable Application Form and send application information by E-mail to mba@up.edu.ph. The instructions for submission of applicant information by e-mail are enumerated in Section B below. Make sure you strictly follow the online submission procedure in this section. You will receive an acknowledgement receipt by email if your submission was successful.
     
  2. Go to the MBA Program Office at least 3 working days after your online submission to submit other materials and pay the required fees. Your application materials and payment will not be accepted if your online submission in stage 1 above was unsuccessful.
     
  3. Take the UPCBA Graduate Program Admission Test (GPAT) on the scheduled date.
     

B. Instructions for e-mail submission of application information
 

  1. Download the latest version of  Adobe Reader (version 9) and install in your computer.
    PLEASE NOTE: Most of the old versions will not be able to save the information you will type in to the application form.

     
  2. Download and open the Application Form using the reader. 

     
  3. Provide (type in) the required information.

     
  4. Click on the "Save" button located at the upper left corner of the form. 
    (NOTE: This save button of the application form must be distinguished from the save button of the browser or that of the Adobe software.)

     
  5. When the "Select Email Client" choice box appears, select "Internet Mail" and click on 'OK'.

     
  6. In the window that appears next, accept the default filename, Masters_application_form.xml, and click on "Save" to save the information you entered in the form. 

     
  7. Send the file that you saved by e-mail to mba@up.edu.ph.
    (NOTE: Do not send the PDF file itself, just the XML file you saved in step 6.)

 

 

 C. Requirements for Admission

The documents and materials required for admission are as follows:

  • Hardcopy of a duly accomplished Application Form
    (must also be submitted online; see Section B above)
     
  • Original Transcript of Records
     
  • Latest certificate of employment indicating your duties and responsibilities
     
  • 3 copies of 2x2 photograph
     
  • Photocopy of NSO-certified birth certificate (original to be presented)
     
  • Photocopy of NSO-certified marriage certificate (if married, female; original to be presented)
     
  • 2 letters of recommendation (to be sent by recommender; Recommendation Form)
     
  • Payment of Graduate Program Admission Test (GPAT) fee of PhP600.00 for Filipino citizens and US$50 for foreign applicants; Application processing fee of PhP150.00.
